ACUERDO DE VOLUNTARIADO Y FORMULARIO DE SEGURIDAD
¡Bienvenido/a! Gracias por compartir su tiempo y habilidades con Conexión Latina Newport (CLN). Por favor, lea este formulario detenidamente antes de firmar.
1. REGLAS Y RESPONSABILIDADES DEL VOLUNTARIO
• Información verídica: Prometo que la información que proporciono en mi solicitud es verdadera y completa. Entiendo que dar información falsa puede dar lugar a la terminación inmediata de mi trabajo como voluntario.
• Estatus de voluntario: Ofrezco mi tiempo como voluntario/a. Entiendo que no soy un empleado/a y que no recibiré pagos, beneficios de salud ni promesas de empleo.
• Reglas y representación: Acepto seguir todas las reglas de CLN. Entiendo que no estoy autorizado/a a hablar con los medios de comunicación ni a firmar contratos en nombre de CLN sin permiso.
2. VERIFICACIÓN DE ANTECEDENTES PENALES (BCI)
• Algunos roles de voluntariado (como trabajar en las Escuelas Públicas de Newport) requieren una verificación de antecedentes penales (BCI) por parte del departamento de policía.
• Usted es responsable de completar el proceso de BCI.
• El departamento de policía cobra una tarifa de $5 por el BCI. Si esto representa un obstáculo para usted, por favor hable con un miembro del personal de CLN.
3. CONFIDENCIALIDAD DE LA INFORMACIÓN DE LOS CLIENTES
Conexión Latina Newport toma muy en serio la protección de la privacidad de nuestros clientes, la comunidad, el personal y los donantes.
• Información privada: Esto incluye nombres, familia, números de teléfono, dirección y cualquier información personal o identificativa.
• Mi promesa: Mantendré privada toda la información de los clientes y de la comunidad. No compartiré, publicaré ni hablaré sobre información privada de los clientes con nadie fuera de CLN, incluidos familiares y amigos.
• Cuándo se permite compartir: La información solo se puede compartir con el personal de Conexión Latina o con el permiso expreso del miembro de la comunidad Y de Conexión Latina.
• Violación de las reglas: Si rompo esta regla y comparto información privada sin permiso, mi voluntariado con CLN terminará de inmediato y podría enfrentar consecuencias legales.
4. SEGURIDAD, EXENCIÓN DE RESPONSABILIDAD Y ATENCIÓN MÉDICA
El voluntariado puede implicar actividades como levantar peso, caminar, viajar o ayudar en eventos al aire libre.
• Comprensión de riesgos: Sé que el voluntariado conlleva algunos riesgos, como lesiones accidentales, enfermedades o pérdida de propiedad.
• Exención de responsabilidad: Elijo ser voluntario/a de manera voluntaria. Acepto que no responsabilizaré legalmente a Conexión Latina Newport, a sus líderes, empleados u otros voluntarios en caso de lesión, enfermedad, muerte o pérdida de propiedad mientras realizo trabajo voluntario o viajo hacia/desde los eventos de CLN.
• Gastos médicos: CLN no proporciona seguro de salud para los voluntarios. Soy responsable de pagar mis propios gastos médicos si me lesiono mientras realizo trabajo voluntario.
• Tratamiento de emergencia: Si tengo una emergencia médica durante una actividad de CLN y no puedo hablar por mí mismo/a, otorgo permiso al personal de CLN para solicitar ayuda médica de emergencia.
5. CONSENTIMIENTO DE FOTOS Y MEDIOS
Al realizar trabajo voluntario, usted permite que Conexión Latina Newport (CLN) le tome fotografías o videos. Utilizamos estos materiales en nuestro sitio web, redes sociales y boletines informativos para compartir nuestro trabajo, sin compensación económica.
• Fotografías posadas: Siempre tiene el derecho de decir "No, gracias" si alguien le pide que pose o se coloque para una foto.
• Fotos de fondo (Su responsabilidad): Tomamos fotos en muchos eventos y actividades. Si no desea aparecer en las fotos, es su responsabilidad informarnos ese día que no desea estar en las fotos y alejarse o salir del área de la toma.
• Fotografías grupales y de fondo: CLN no puede controlar, eliminar ni editar a las personas que aparecen en el fondo de fotos grupales o de eventos. Usted acepta que CLN no es responsable si su rostro aparece en el fondo de una foto o video.
6. IGUALDAD DE OPORTUNIDADES Y NO DISCRIMINACIÓN
Conexión Latina Newport le da la bienvenida a todas las personas. No discriminamos por motivos de raza, color de piel, religión, origen nacional, estatus migratorio, edad, identidad de género, orientación sexual, discapacidad o cualquier otro estatus.
7. CORREOS ELECTRÓNICOS Y FIRMAS DIGITALES
• Al proporcionar mi dirección de correo electrónico a continuación, acepto recibir horarios de voluntariado, actualizaciones y noticias de CLN.
• Acepto que firmar mi nombre electrónicamente (o marcar una casilla en línea) tiene exactamente la misma validez legal que firmar en papel.

1. VOLUNTEER RULES & RESPONSIBILITIES
• Honest Information: I promise that the information I give on my application is true and complete. I understand that giving false information may result in ending my volunteer work immediately.
• Volunteer Status: I am offering my time as a volunteer. I understand that I am not an employee, and I will not receive payment, health benefits, or job promises.
• Rules & Representation: I agree to follow all CLN rules. I understand that I am not allowed to speak to the news media or sign contracts for CLN without permission.
2. BACKGROUND CHECKS (BCI)
• Some volunteer roles (such as working at Newport Public Schools) require a criminal background check (BCI) from the police department.
• You are responsible for completing the BCI process
• The police department charges a $5 fee for a BCI. If that poses a barrier for you, please speak to a CL staff member.
3. KEEPING CLIENT INFORMATION PRIVATE (CONFIDENTIALITY)
Conexión Latina Newport is very serious about protecting the privacy of our clients, community, staff, and donors.
• Private Information: This includes names, family, phone numbers, address and any personal or identifying information.
• My Promise: I will keep all client and community information private. I will not share, post, or talk about private client information with anyone outside of CLN, including family and friends.
• When Sharing Is Allowed: Information may only be shared with the staff of Conexión Latina or the express permission of the community member AND Conexión Latina
• Rule Violations: If I break this rule and share private information without permission, my volunteer work with CLN will end immediately, and I may face legal consequences.
4. SAFETY, WAIVER, & MEDICAL CARE
Volunteering can involve activities like lifting, walking, traveling, or helping at outdoor events.
• Understanding Risks: I know that volunteering carries some risks, like accidental injury, illness, or lost property.
• Waiver of Liability: I voluntarily choose to volunteer. I agree that I will not hold Conexión Latina Newport, its leaders, employees, or other volunteers legally responsible in the case of injury, sickness, death or loss of property while volunteering or traveling to/from CLN events.
• Medical Bills: CLN does not provide health insurance for volunteers. I am responsible for paying my own medical expenses if I am hurt while volunteering.
• Emergency Treatment: If I have a medical emergency during a CLN activity and cannot speak for myself, I give CLN staff permission to call for emergency medical help.
5. PHOTOS & MEDIA CONSENT
By volunteering, you allow Conexión Latina Newport (CLN) to take photos or videos of you. We use these on our website, social media, and newsletters to share our work, without compensation.
• Posed Photos: You always have the right to say "No, thank you" if someone asks you to pose or stand for a photo.
• Background Photos (Your Responsibility): We take photos at many events and activities. If you do not want to be in photos, it is your responsibility to let us know that you do not want to be in the photos that day and step away or move out of the picture area.
• Group & Background Pictures: CLN cannot control, remove, or edit out people who appear in the background of group or event photos. You agree that CLN is not responsible if your face appears in the background of a photo or video.
